The BLACK Collaborative Inc. – Access to Care Initiative
Purpose:This form allows individuals and families to self-report their current needs, request support from the Community Care Closet, and share relevant background information that may inform referrals to partner programs such as BLACK BERRY (perinatal mental health) or the B.L.A.C.K. Rhythm & Blues Initiative (maternal cardiovascular health).Completion of the trauma screening section is optional and is used only to guide connection to supportive services.
