What We Offer
Lactation / IBCLC Support
Breastfeeding and chestfeeding help, pumping plans, flange fitting, return-to-work support, and NICU transitions.
Doula & Perinatal Support
Birth planning, labor support, postpartum recovery checks, safe sleep, and newborn feeding guidance.
Parenting & Fatherhood Engagement
Parent education, father/partner involvement, developmental milestones, and strengthening home routines.
Community Care Closet
Diapers, wipes, formula, clothing, hygiene kits, and baby gear based on available inventory and family need.
Social Needs Navigation
Assistance with food access, housing, utilities, transportation, and community resource connections.
Mental Health & Medicaid Navigation
Warm handoffs to counseling or therapy, postpartum mood support, and help applying for or using Medicaid.
How It Works

Submit the referral form: Families, Community Health Workers (CHWs), social workers, and partner organizations can complete the form below. Please include the best way to reach the family and indicate the urgency.

Triage and routing: Our team reviews each request and routes it to the right program—Maternal-Infant Health, Behavioral Health, Social Needs, Youth Services, or the Community Care Closet.

Follow-up: We reach out by phone, text, or email. High-urgency requests are prioritized. Some services depend on funding, eligibility, or inventory.

Some services are virtual or dependent on available resources. If you live outside our service area, we’ll do our best to connect you with a local partner.

For Partners & CHWs: Community Health Workers and referring organizations should include their contact details in the form and select “Notify referrer after outreach” if they’d like updates once we connect with the family.

Is there a cost?
Most services are free or grant-funded. If a fee applies, we’ll discuss options and a sliding scale before scheduling. -
Do I need insurance?
No. We can help you apply for or use Medicaid, but coverage is not required. -
Can a CHW or social worker refer me on my behalf?
Yes. We ask referrers to confirm client consent before submitting the form. -
How do I request items from the Community Care Closet?
Please let your support member or case manager know. Items are provided based on current inventory and funding. -
Do you serve fathers and partners?
Absolutely. Our programs include father/partner engagement and parenting support.
